Include with your application:
- One passport-sized photo (with name printed in ink on the back)
- Payment (if applying by mail, include delivery costs as shown below)
- Proof of student, faculty, or youth status as shown in the eligibility
categories above
- Proof of age (either a copy of your driver's license or passport)
Delivery Options (if applying by phone or mail):
- Standard Handling Fee - $3.00 Upon receipt of application, allow 4 weeks
for delivery.
- Rush - $15.00 Send application via Airborne Express. Upon receipt,
allow 5-7 business days for delivery
- Express - $25.00 Send application via Airborne Express. Upon receipt,
allow 2-4 business days for delivery.
